Where are you located and what sort of insurance does he have? Does he sleep well at night?

In Brooklyn.
United healthcare
He was sleep trained very young. (Happened by mistake, cuz he couldn’t fall asleep and I knew nothing is keeping him back. He was bathed, fed, room dark, white noise machine... but couldn’t fall asleep, so just walked out of his room and he was sleep trained)

This doesn't sound normal and at his age he shouldn't still be needing formula. Ditch your doctors and find a new one, maybe one a little more alternative or open minded who will help you get to the root of it.

See if you can get into Dr. Wetzler at Maimonides. She was a good shaliach for my daughter after lots of doctors (even a renowned specialist) failed her.
By us it was the lack of sleep that led to her first diagnosis. But you need to speak about how cranky and hungry he is but won't eat and is just miserable. Otherwise they'll go by the number on his charts and assume he's happy.

You mentioned he's getting OT. Is he seeing a feeding specialist as well?
I know you're exhausted by all this, but the squeaky wheel gets the oil. Don't be afraid to be "that mother", and keep pushing until you find the right doctor.

what ra_mom said. if he's acting hungry and not eating, there's a good chance something is physically wrong.
