I want to tell someone that what they think of me is all wrong. What's the correct wording- "you have a wrong perception of me" or "your perspective of me is all wrong"?

I would go with, "Your perception of me is wrong."

Consider making it less confrontational. Steering away from the words, wrong and you makes the sentence less accusatory and will possibly cause the person to less likely be defensive.
Some examples,
I think that there may be a misperception of me.
My ______ (words/actions) might not be perceived as I intend them.
