What can I do? I feel very helpless. My son wakes up in the morning, pulls off his pull up and continues on his day. Every morning or after bath it's a fight to get him to put on his underpants. I know I am not dealing with it properly. I have a history of s-x trauma and I can't handle seeing him that way. I yell at him etc...I feel so lost and scared he will grow up...I don't even know what I'm scared of. What would be a calm method of discipline?

Are you in therapy? Is he special needs?
If a normal kid, you can take away privileges.

It's very hard having a special needs child that age. I empathize.
Ask his therapist for suggestions. We don't know what his special needs are so ite hard to tell where the issue is.
