The good news is that if it’s pandas it can be fixed (at least temporarily) with antibiotics. See if you can find a pandas aware pediatrician.

I'm located in lakewood. I'm using precious health, Dr. Werczberger. I myself am a successful OT bh but I can't seem to help my own child

On days when it was too rainy or too hot, I would take DS to Petco. All the staff knew us.
We let DS run up and down the aisles. He loved the dog and cat toys that made all different sounds, had different textures, etc. We'd let him explore to his heart's content, and when we sensed that he was winding down, we'd take him to look at the fish. Watching the fish swim around was very soothing for him.
Hashem help us if we skipped a day of stimulation, though! It was non stop motion. B'H, eventually we found the right medication and he's doing somewhat better now. ABA helped a lot, too.
