It’s in my car from milk leaking (I don’t know where it spilled) and I tried washing with vinegar and water but still bad smell. Any ideas?

You must find the source, otherwise nothing will do. If you don't find the source, the odor will eventually dissipate on its own. You have NO idea where it spilled?

You'll probably need an enzyme remover to remove the scent, since milk is a biological excretion. But you'll need to find the exact location where it may have spilled, since the spray will need to get to every part of the spilled milk

It needs deep shampooing. You need to find where the spill is and shampoo and rinse away.

Not to be pedantic (OK, it IS pedantic) but milk is a SECRETION, not an EXCRETION. Excretion is the process of eliminating or expelling waste matter, which milk is not, in any way, shape or form. Secretion is the process by which substances are produced and discharged from a cell, gland, or organ for a particular function in the organism.
Please apologize to nursing mothers everywhere.
