Coffee tastes off

I don't know where to put this so I put it here.
I go out once a week with my dh, for a coffee. We always go to the same place. Lately we switched to another place and we really enjoyed the atmosphere there. Anyway a week ago my dh ordered a second coffee and kept on saying it has a weird taste. But he drank it anyway. He loves coffee and can't resist it. He took a sip and put it back saying it's not good, but every few minutes he took another dip even though he didn't like it. when he had drank the half, he noticed that the coffee wasn't clear. It has white pieces inside. He showed it to me and I smelled the coffee and it stank. Almost like there was sigarette buds inside. When he showed the manager, he said he doesn't see anything it's a normal coffee and bla bla bla. When we went out one of the waitress said that if you let the coffee sit to long it can happen.
A week later we went again. My dh didn't want to go there but I said we always enjoyed there let's forget what happened. We ordered a coffee it was fine. After the meal my dh ordered a second coffee and it was disgusting again. We took pictures of the coffee. You can see it's totally full of white pieces and it smells really bad. My dh didn't say anything this time. Obviously we are not going back there again...
My dh was so disgusted. He wanted to throw up. He was really scared of what it is that he drank. Anyone ever had this? Do you know what it can be. I thought maybe it's the last water in their kettle and all the stone web into my dhs coffee. I also said that to the manager the first time. They would have cleaned it no?

Was there milk or creamer in the coffee? Spoiled milk will cause this

I can tell you exactly what it is. I used to work in restaurants.

When they keep coffee on the warmer too long, it gets slightly burnt, and it evaporates down. Instead of dumping it out, they just brew new coffee on top of it. Even worse, if the manager is cheap, he makes you serve the burnt coffee before you are allowed to brew a fresh pot!

I learned that a lot fewer people order decaf, so that's usually the pot that goes stale and burnt first. If you ever order decaf, make sure you ask for a fresh pot.

If you are at a place that serves espresso, even if you just want a normal coffee, order an Americano. It's plain coffee, but it's brewed fresh just for you.
