There is a gene that people with celiac disease have that causes the intolerance. As far as I understand, a person can have the gene but not have "active celiac disease" (I'm not sure how to explain it properly). So the blood test can show the presence of the gene but the person might not actually be intolerant to gluten so the biopsy would be clean.
I'm not a doctor, and I'm not sure if I'm providing a clear explanation, but that's basically what our GI told us.
