My 18 year old son doesn't have a name on his birth certificate and now wants to get permit anyone have a idea what to do?

How was it possible to not have a name on hai birth certificate? I thought all hospitals worldwide required something

So what name is on it?
It's it just your surname? Like he only had one legal name?
How did this question never come before with registration, insurance or doctors?

Everything is filled out besides his first name

You need to make an official name change.
Call your local office that deals with birth certifcates and they will have you fill out a form.
It might vary by state, but I recently had a similar issue with my son. He was listed as boy and the surname. I had to change the name in order to apply for a social security number.
Op does your son have a ss number?
