Call up Vanguard and they will guide you through the process. Fidelity is another good option in terms of low fee brokerage options as both Vanguard and Fidelity have no low/lower fee mutual funds.
As to the rest of your questions, you would do well to get a very basic book on investment so that you can make informed decisions and evaluate the advice anyone gives you.
