What exactly do they do that gets the baby to start talking?

Depends on the kids age, issue and type of therapy.
If it's a oral motor issue, they do excersizes to strengthen the muscles.
If it's apraxia where the kid can't figure out how to get their mouth to make the sounds, the best thing is prompt which teaches them how to move their mouth.
If it's a language delay, they'll work on stimulation language through play
