I received a bill from periodontist for periodic evaluation and perio maintenance, 2 different procedure codes. One is for cleaning and one is for dr. to check gums and pocket depth. Dr. was sick on that day so he did not evaluate. Can he charge for evaluation and say hygienist did the evaluation?

Likely it was in the schedule for the day and they forgot to take the codes out when putting your visit through to insurance. Just call the doctors office and ask.
