First of all, in the box is totally fine too. As long as he feels it is his special day and everyone made it special. Don't add extra pressure with the need to be "out-of-the-box". The main thing is leibedig.
In my opinion there are three things that make the bar mitzvah:
lively dancing, good speeches, and good meat. Everything else is commentary.
You can have each friend/relative come up to the podium and give a custom bracha to your son based on a keyword they draw from a hat.
Have a friend/relative write a song/dance that everyone will surprise son with.
Have someone that is a good speaker give a memorable speech.
Print out lots of pictures of son over the years, string on a rope and hang by the dessert tables.
Prepare a how well do you know Moishe type game for everyone or just the friends to play. Can use a powerpoint with pictures for the questions.
