If all they did is sell your chometz, maybe $10? $18? We give much more because we are giving in thanks for all year where they helped us with shaalos, answered our questions, etc.

Same. It's a good time for a tip. I think some payment is necessary for the sale though.

Dh selling to same dayan he uses all the time. So probably giving around $200. As appreciation tip, as well.

Never ask him any shailos or anything but he’s a really nice rav…. I gave him $100 (definitely the upper limit of what I can afford).

If you are not affordable you can give a few dollars too. Our previous Rav would not alow kollel yingerleit or others he knew were hard up, to pay more than the equivalent of $1.

It is NOT a requirement to pay.
(BOTH my father and my husband sell chametz.)
However, it is a time of year that people will show appreciation to the Rav.

Nope, its not a thing where I live.
The rabbi has a form that people put their names, phone number and address down, during hagala times next week. It takes 3 seconds and thats it.

Really interesting, we just discussed this because I always assumed yes. DH told me not only is it not a requirement, the online form actually has no option for it!
(Our hakoras hatov is at Purim).

This isn’t accurate. When you do a shtar with anything you need to give over some sort of compensation (could be $1) but there needs to be an exchange of money to make it real and assertive. My husband gave the Rav $20 last night and the Rav thanked him and said it’s the proper way to make a shtar but he obviously cant sit there and tell people that because he’s asking for money.
