My son has a similar profile. What was most helpful for us was a private case manager who was in contact with the teachers and school about his abilities and needs, went down to school to see how he was doing, guided them in what to expect from him and how, identified which needs were most important to meet in academic, developmental and social areas and helped find and coordinate providers. I'm in NJ and unfortunately there isn't much that is covered. Although that may be different now as there are many more aba based options available if that is a label you also choose to pursue.
To give you hope, my child is a teen and doing well for the most part in mainstream school with some minor Kriah and social weaknesses. But considering where we have come from it is truly a miracle.
Medication was also a game changer for us as it enabled him to more easily learn the skills he was being taught
