I think I heard that you don't ask specifically unless there's a tefila for it (like in u'nesaneh tokef, avinu malkeinu, a special yehi ratzon, etc)
But you can (and SHOULD) concentrate and have kavanos for all the things you want as you say words that relate to them.
There's a common ad I've seen every year that says during mussaf, when we say "Mi lo nifkad bayom hazeh" (or something like that, not looking it up now) that you should have kavanah for having children, as Sarah Imenu was "nifkad" on Rosh Hashana and gave birth to Yitzchok Avinu that year.
Kavanah is everything.
