Does he speak at an age appropriate level? It’s often a sign the kid is frustrated and unable to communicate properly. I’d do a speech and language processing evaluation.
Did he always behave like this or a recent behavior change? Sudden aggression is a sign of possible Pandas. Go to a specialist that is familiar and treats this. Your regular pediatrician will just give a throat culture and if it's ongoing pandas the strep will not come out positive.
