The key for me is to recognize my triggers and try to prevent them from triggering me; and when it is too late, removing myself from the situation before I yell.

I guess I am skipping a step. In order to remove yourself, you need to recognise what you are feeling IN the moments leading up to the scream.

Can you break it down in the moment?
Consciously walk yourself through (he stages up to and during your yelling? Then, during another stressful moment of your day, remove yourself from the situation without warning --- just leave. Go into your room, close the door, lock it if necessary. Tell your children you are in time out. They will understand. They will slowly see and appreciate the changes you are making for yourself --- and you will eventually see the change happening in your family dynamics at home.
Hatzlacha!!! You got this! Hug
