Take him to a pediatric urologist. It might not be the inverted thing, since it would probably pop out when the urine starts flowing. It might be that the hole at the tip is either too small or not in a perfect place. DS had it. He was spraying the ceiling! One visit to the dr., a local anesthetic cream, a snip, some vaseline to keep it from closing up, and his aim is perfect now!

can you teach him to punch it out himself?

they make these things called piddlers toilet targets that help with aim.

I was in TJ Maxx today and saw a plaque:

My aim is to keep this toilet clean. Your aim would help.

We had problem witth spraying/bad aim until we got the idea from someone to put a table tennis ball in the toilet bowl which he now aims at and which he thinks is a fun game.
We tried the stickers which are designed for this before we tried the table tennis ball but they didnt stick well enough and fell of within a few hours.
The good thing about the table tennis ball is that they you can get very cheap ones as well.

I have 7 DS and when they were growing up we had a hand painted plaque (painted by yours truly) hanging in the "boys" bathroom over the toilet Smile

It stayed until that bathroom became a "guest" bathroom and it wasn't appropriate to leave it there.

I think my DD took it for her house.
