What to write for a teacher- can you help

I'm looking for short, sweet , few sentences to write to a teacher. a thank you & appreciation for all the hard work...

writing is not my strong point, can you help?

Write from the heart. You'll be fine.
Here's a start.
Dear Teacher,
Thank you for all you do for (kid's name). S/he has blossomed this year thanks to your (list a few good qualities). We still talk about (project, activity, etc.)
"Vechol me sheoskim b'tzarchei tzibbur" (write in Hebrew. Or give your own bracha.)

Your name

Teachers have told me how much they appreciate a note from the heart. And yours is clearly in the right place. Don't worry so much about the "write" place.

When you write the posuk, is it written all out or just the part you posted here?

It's at the end of the Mishebeirach right after the Yekum Porkans.
