It's short for not 100% normal.

I'd rather be described as not 100% than as a wackadoodle. Wackadoodle is a humorous word but clearly means "total nut case". "Not 100%" means a little off in some way but not necessarily a total nut case. And I would disagree with the poster who feels that "not 100%" is judgmental while "wackadoodle" is not. Punk farkert. "Not 100%" is a euphemism for "something a little bit wrong with him" while "wackadoodle", humorous though it may be, is definitely a pejorative. Possibly even an affectionate pejorative, but a pejorative just the same.

A few bulbs short of a chandelier.
The butter slipped off the noodles.
The elevator doesn't go up to the top floor.
A sandwich short of a picnic.
Not the sharpest tool in the toolbox.
Been microwaving with the door open again.
Not firing on all cylinders.

It’s more of an opinion, then a judgement.
“That lady is going around the US harassing all the Muslim candidates for Congress, she’s a complete wackadoodle” (an eccentric or fanatical person)
“Yup, from what I can see she’s not 100%.”
The way I see it the first sentence gives an opinion of what she doing, and the second comments on her mental capacity.

Is it "not the sharpest crayon in the box" or "not the brightest"?

It’s a great word, right up there with flabbergast, flibbertigibbet, and hornswoggle. Can’t use any of them in official correspondence, more’s the pity.
