My 4 year old woke up with bad itching in diaper area. I tried nystatin but she keeps waking up and crying that she's still itchy. What can I put on to stop the itch?

Do you see a rash? I second pinworms. Especially bec you say she is complaining at night.
Edit- I ‘third’ pinworms

I did, didn't see anything. She is red in the inner part in the front. She was complaining about her stomach hurting today. Could it be a UTI?

It is most likely pinworms.
You can try giving her a quick bath to rinse off the residue left there, which can cause the itchiness, it’s not just the actual worms

If it's pinworms wouldn't she be itchy in the back. She's only itchy in the front?

Pinworms can cause stomach aches and diarrhea too. They crawl into the v@gina. Just last week I found it in there in my toddler.
