base of bed isn't wood - it's heavy cast iron with wheels and a furniture headboard attached
are linen stores still doing those?

Can you cover the box spring with an encased sheet so you don't have to use a dust ruffle?

I've never used a dust ruffle in my life. As far as I'm concerned, a dust ruffle is just that--a shmatte to collect dust and make life difficult. They do not keep dust from collecting under the bed, so why bother?

what size for 48 inch beds?

Yes, I put it on the box springs. It kind of looks like the box spring is covered with a sheet. But it's very snug and fitted. It's a more modern look (than dust ruffles) and is mostly covered by the quilts. My DD's (who know everything modern better than I do) were the ones who instigated this look.
