In my specific case, it's an engagement party for a very close family friend from my parents, meaning their friend's grandchild. They have done special things for us and I feel close to and indebted to them.
I'm not feeling awful, but it's a 50 minute drive and I would have to drive myself and find parking. I feel like it might put me over and make me actually sick. Obviously they would understand if I said I wasn't feeling well, but I feel like it just wouldn't sit so well.

Can you get a ride with someone else? Can you restructure your day to make it less burdensome?
If you don’t go, can you call (not text) wish them a heart felt mazel tov, extend your apologies and send a gift with a beautiful card asap (either from a registry or handmade/hand selected).
Only you know how you really feel. You don’t need to make yourself sick but you do need to express your joy in their simcha.

You don't go to a party sick, period. You think you'll be doing anyone a favor by attending? How do you even know that whatever you have is not contagious?
