Usually you can get for free through your kupat cholim in hitpatchut hayeled. Which kupah are you? Maybe I can help you with the process.

It has become shorter over the years - they have made new policies about wait times.
If it can wait a few weeks, go through the kupah.
If there was an injury and you need it sooner for rehabilitation, you can try through the hospitals and request hitchayvut from the kupah.
If you need something immediate not for rehabilitation for an injury, you'll have to go private, I'm sorry I don't have anyone to recommend.

Request a referral (hafnaya) from your pediatrician. I like to call and ask what is needed - forms and/or other things. Usually just the hafnaya and forms; it probably isn't necessary to call. You will need to fill out the forms either way - There are usually a form for the parents and one for the ganenet. You can do it directly online or send them in to hitpatchut hayeled (there is a big one in Givat Shaul. I think there is also in Gilo). At that point, I would call to confirm they received them, ask them the specifics but I believe they need to get back to you within a certain number of days to make an appointment.
