I’d say rabbi and rebbetzin first and last name. What’s a title? Like breslover rebbe…? I don’t think that’s done.

That’s to the rabbi when you go in before simcha but then you send one to house for rebbetzin

I dont remember how we addessed it but I know we send 2 diff ones. One for rebbe and one for rebitzin .
We did that according to the list from kehilla

Who is grand and who is just rabbi?
OP I think you'll have to post the Rabbi's name and we will decide if he's just simple Rabbi or he's worthy of the title grand.
Jokes aside you write Rabbi and Rebbetzin and you send it via USPS.
No hand deliveries necessary and unless he is one of the famous rabbis with hundreds of chassidim you don't write Grand.

If you want the Rabbi to actually come, you usually do go invite personally with an invitation.
You would still send an invitation in the mail so the Rebetzin gets invited.
Rabbi or Grand Rabbi depends if he has hundreds of chassidim
