Should a three yr/o typically understand when you tell them they can’t massage your chest in public? And how to gauge if they’re acting more babyish than is typical?

Not necessarily and just because they understand they shouldn't do it doesn't mean that they'll actually stop doing it.

I don’t think it’s a concern yet. But if she can’t understand public I’d also stop it in private.

This. You shouldn't be allowing it in private either.
You need to redirect her/move her hand away. Rinse and repeat. Till she gets it. Which may take a while.
