I'm trying to make up a quick reference guide for my babysitters.
I think I got most of the pertinent info:
Parent's names, cell numbers, work numbers, pediatrician & phone#, birth dates, hatzala phone #, exact address, pertinent medical info, and grandparents' phone numbers.
Anything else I should be including? Anything that is superfluous?

Make sure your address is written down somewhere!
I had the exact same sheet prepared for my babysitter and asked her if it was missing anything and she pointed that out. I don't know about you, but here in LKWD, I tend to pick up my babysitters and, even when they walk over, they know it for the first time and then just remember how to get to you from then on (without committing the actual address to memory). If something chas v'shalom happens, you don't want to waste valuable time while she runs outside to check the numbers on the door
