Looking for a sister-of-the-bride gown, plus size. Are there any stores in Geulah or around Yerushalayim that might have? Or even a gemach?

I have a plus size friend who got a dress for her son's bar mitzva at a place called Nefertiti in Kenyon Hadar in Jerusalem. They specialize in evening wear.
She looked very regal at the affair.

Do you think that a regular store in Geula would sell gowns in a size 48? Or would I have go to a place that specializes in plus-size?

Sorry, I really have zero experience with stores in Geula so I don't know.
I think that re other stores and designers 48 is sort of borderline.
You wouldn't necessarily need a plus size store for that size but OTOH not everything might be in stock in that size in a regular store so there might not be much selection.
Maybe someone who knows the stores in Geula can chime in?
BTW I think there might be some stores in the Center One area that have formal wear in plus sizes.

Onot has great plus-sized clothes but I don't know what they have in terms of formalwear. In the past I've ordered dresses online from Macy's, which has gorgeous, true-to-size, plus sized gowns. Delivery to Israel isn't cheap but is possible. The dresses themselves tend to be reasonably priced at least.

You can go to Tali Tal on King George - near Yaffo.
I got all my mother of chatan/kalla outfits there.

There's a place called Koresh on Yafo (a little bit down from the ben yehuda train stop). They are not cheap but they carry fancy gowns and have some in larger sizes - you probably won't have tons of choice there but I got lucky finding something for my sister's wedding there. Also a lot of their stuff is imported from America so you could even get an idea of fit in certain brands and then look more online.
