Or you could do a SCD diet cake, which is made out of almond flour. I've had Wellbee's muffins before and they are delicious, just like the real thing, but no carbs.
They have their cookbook on their site, as well as all ingredients. And they are heimishe. You can buy products straight from them, though I don't think cake is something the stock. But you can ask them.

Does it have to be a cake? Make a nice dietetic dinner for him and stick a candle into any dish.
Maybe plate a colorful stack of grilled veggies and stick a candle on top. Oh, or meatloaf if he can eat that on his diet.

When I was on a diet my friends surprised me with a rooftop dinner consisting of healthy food with a huge fruit salad for dessert. It meant so much to me that they took my diet into consideration.

Sushi cake. You can use brown rice in springform pan with layers of rice, nori, Kami,vegetables, and drizzled with spicey mayo on top

Zuchinnie brownies with a frosting that uses honey or a natural sweetener instead of sugar.
Tastes like a regular brownie, and you'd never know there's vegetables in it as the base! (Think carrot cake).
