Appreciation words to engrave on a gift

I'm giving my therapist a gift and I'd like to engrave a few words like ,"with utmost appreciation" or "forever indebted". I can't decide on a good one. Any ideas?

Also, I wanted to engrave a nice long sentence but the saleslady told me that people usually write that on a card and engrave 2-3 words and the name. Any advice?
