Did you try allergy meds on a regular basis?
I would try that first.

We tried Flonase, didn't help.

Which allergy meds?

I would take them off dairy and test for allergies first. That’s the culprit most of the time

She is actually off of most dairy (mild lactose intolerance) except yogurt. Could that still be causing it?

I would try less invasive interventions first:
Clean up diet, remove dairy and gluten
Run food and environmental allergy testing
If you know you have mold, take care of that
Air purifier for environmental allergens
Daily anti histamines if it's allergy related
Check for tongue tie and other myo-facial or airway restrictions
Check for enlarged tonsils and or adenoids, if present try to address what's causing that
Craniosacral therapy
Chiropractic adjustments

Check with an ENT. This may be an adenoid issue more than an ear issue.

Can my regular pediatrician do allergy testing, or do I need to go to an allergist?

Why would gluten cause a runny nose?

PS pediatrician claims she is too young to have seasonal allergies (even though I have them so its definitely in her genes) Confused

We've been to an ENT, he was the one who put her on Flonase (she has mild fluid in one of her ears). Didn't mention anything about adenoids.

The follow up visit is this week I'll ask about adenoids

Yup, I’d think adenoids before ears.

Runny nose is a common reaction to food intolerance, and gluten is a common culprit.

My allergist said that testing doesn't always show up under age 5. She recommends xyzal, finds it's more effective than some other OTC allergy meds

My son has some he got his first set of shots. This after every cold / virus I need to boost his immune system for a few weeks. If I don't his nose just runs indefinitely. His immune system has weakened.
(Yes he's off dairy mostly. Other than shabbos a bit of a treat) helped a lot. But still needs immune boosters for a month minimum after every cold/virus.
Have you tried regular immune boosters, echinacea, Panto c / other immune boosters?

We do nightly Zyrtec plus Flonase as needed.

Tubes will only help if it’s backing up to the ears

I've tried probiotics. Which other supplement would you recommend to try first?

This is true. But I'd be very wary of taking a growing developing child off gluten.
Oftentimes, there are other things to do to help the body tolerate gluten/dairy better while limiting dairy/gluten.

Speaking as a mother who is mostly gluten free with one child off wheat (she eats spelt) and one child gluten free. It ain't so simple.
If you do think it will benefit your child, discuss it with your pediatrician or a good child nutritionist first. Do not do this lightly.

Prebiotical and probiotics aren't enough.
Try zhalers or maxi immune boosters for 3-4 weeks and Panto c. They taste good so kids like taking them. Mom just had to make sure to remember. Then reevaluate.
I'm sure there might be better options than these two. But these 2 have worked for my kid in the past.
For an older child who can swallow I'd do low dose maxi biotic then taker off one pill every week.
