DS is 20 months, and his scrotum is all red. He screams when his diaper is soiled. There is no other rash present on his butt or area. Does he need a doc are is it enough to try diaper rash ointment?

its probably diaper rash, maybe even yeast/fungus. put cream on and if isnt better in a day or 2 take him to the dr.
