I know a Tzvi Hirsch and Zehava Gold.
They were both named after people with the Yiddish name but they're parents felt that Hirsch Hirsch or Goldie Gold is just too much so went with the Hebraicized version.

I know someone Avrum Yitzhak Abraham. He goes by Isaac Abraham. If not it would be Abraham Abraham.
I know a Hershy Hershkowitz, and a Hershy Hirsch.
Kalman Yakov Kolman. She wanted to call Yanky, but zeida was called Kalman, so family insisted he called kalman. Zeidas last name was not kolman. In beg she went by both names, but at home, she calls him Kalmen, it's easier. So now he is Kalmen Kolman.
