How do you reward a child for an exceptional grade on an exam (and the obvious huge effort that went into preparing)?

You don't reward results; you only reward effort. Achievement would be non-existent at our house if it weren't for Slurpees from 7-11, but every community and family has their own treat.
I've also been successful with cumulative rewards. For example, my youngest son was balking about attending shul regularly. We devised a "contest" with various levels of prizes based on his percentage of attendance. Although I was concerned that his behavior would backslide after the contest ended, it didn't. Once he'd developed the habit of attending shul, he was perfectly fine with it.
But hishtadlus is what counts -- not results!
