You heat the milk until it's warm/hot. Make sure it doesn't boil.

I love hot chocolate and I’ve tried many recipes. This is my favorite one. It’s from the whisk.
Hazelnut Hot Cocoa
1/4 cup sugar
1/4 cup cocoa powder
2 cups half and half
1/4 cup + 1 Tablespoon hazelnut spread
2 Tablespoons chocolate liqueur (optional but recommended)
Marshmallow fluff, to serve
In a medium-sized saucepan over low-medium heat, stir sugar and cocoa for 5 minutes until we’ll combined. Add milk and half & half and bring to a boil. Whisk frequently to keep it from clumping. Add hazelnut spread and chocolate liqueur and simmer for 3-5 minutes. Serve with a dollop of marshmallow fluff in each cup.

I don't have exact measurements, for one cup:
spoonful of cocoa powder
sugar or splenda
pinch salt
1/2 tsp vanilla
stir dry ingredients. Add 1/4 of the hot milk - stir briskly, add the rest of the hot milk.
