They're traveling to Israel. On blue cross blue shield medicaid.
What needs to be done insurance wise? I'm case of emergency?

Most people get travel insurance when they travel abroad. It covers medical expenses when abroad. It isn't meant for chronic conditions but if there is an accident or sudden disease.
It is relatively inexpensive because the reality is that most people don't need it but good if you do need it.
For example, my friend tripped when she was vacationing in Europe and her insurance paid for treatment at the hospital in Europe but most importantly also paid for the very expensive transportation back to the US.
