I ordered from Amazon, looked for something with good reviews. The first pack they sent me a bunch of the labels were stuck together (usable especially with scissors but inconvenient) but they replaced them with no issue.
The ones I put on before camp last year have been starting to fall off over the last month or so, minimum a half year later for the clothes they wear regularly, so I'd call that a success. Not permanent but good enough. I sent an extra sheet or two of labels to camp in case anything important fell off or if we forgot to label something.

I ordered from an ad I saw in a local Lakewood magazine. We picked them up from someones house. Labels I put on 2 years ago are still on even after being washed many times.
Last year I ordered from a different place and some labels are coming off.
The quality of the first one was better.
