You probably want a full consult with a reading/Chumsh specialist. But if I was working with your daughter I would start with something very simple that you can do at home.
Go to chinuch.org and get a list of the most common Hebrew words in Chumush. Make flashcards, and practice reading and translating. What you're trying to do is build up a sight word repertoire. That's most likely the skill she's lacking. It may seem overwhelming, but the 100 most common words make up about 50% of language. If she can read those by sight and translate them effortlessly, she's got the first step towards seeing real improvement.
