I like gag gifts; I'd give a bottle of Mr. Clean in addition to the "real" gift.
But really, how can anyone say without knowing the person? Gender? Age? Stage in life? What's appropriate for a dh in his 40s is not necessarily going to be right for a girl in her teens. An inspirational book by someone else who's been through it? An item of jewelry with an appropriate inscription on the back? Say, a perpetual calendar inscribed " Taking it one day at a time" or a watch inscribed "525,600 minutes of courage" (there are 525, 600 minutes in a year.)
There are companies that sell murals, prints and other items with inspirational messages if you can't think of one yourself.
