Basics like onesies and bibs are often appreciated. Even if it's the third girl, stuff gets worn out, and fresh things are always nice to have.

A new outfit or toy for the new baby is still special.
A special little gift for mommy would be much appreciated! (book, small gift card, manicure, hand lotion...)

I buy baby bath stuff and a cute sponge. You can make a pretty gift basket.

It's a practical gift... you'll need soap, shampoo...etc. for the new baby.

Buy something for MOM!
A good perfume, nice book, or a gift cert in a store.

I do the same the last few years. The nice shampoo, a pacifier (if I know they use them and what type), a teething ring, sippy cup, basically I go in the baby isle of the pharmacy and see what strikes my fancy.

I second the idea of toys for the older children (building blocks, puzzles, crayons & stickers...) . they sometimes feel left out w/ all the excitement about the new baby & where'd Mommy go & all, plus they prob have enough baby toys as those don't get destroyed. You mentioned that you don't feel comfortable getting clothes, but I just want to put out there that depending on season or how much her old stuff has been used, new clothes are prob useful.

oh, & the new toys can keep them busy while Mommy wants to nurse or rest....

Yeah, I was just going to say if her other girls were born in different seasons she's going to need new stuff for this baby.

Something for the Mom would be nice or as someone else suggested, consumables such as bath products and nappies. I always get consumables when in doubt, usually higher end stuff. You could even get them for the mom if she is the kind of person who would like that sort of thing.
