would you let someone know that there is blood on the back of their light color skirt, meaning to say that there period just started or would you not say anything? wwyd?

Unless you don't care if you're walking around in the same bloody situation. Why bother telling her? How did you even notice if you don't seem to care?
This is not about manners or etiquette, it's about common decency. Of course you tell them.

Maybe you could just say-your skirt is dirty, you must have sat in something. (that's a lot less embarrassing to say and hear, IMHO)
