The instigator in a fight comes and complains that his sibling hit him.
Example Chaim hits Levi.levi hits Chaim back. Chaim complains to mommy that Levi hit him

I'd tell chaim that he started it, and I'm not surprised levi hit back. I'd remind levi that there are other ways of getting chaim to stop bothering him. then I tell them that any further fighting is between the two of them, but I'd appreciate it if they play together nicely. I'd then invite them to leave the room I'm in so they can settle things between them.

I'd punish both
One for starting and one for doing nekama
Explain to to both that next time they come to you when they have a problem

If a child got hit I don't blame him for hitting back. One can't be expected to just stand there and take a hit. It don't consider that nekama. I deal with the instigator.
