One of my mother's names is the same as the name of DH's grandmother (with whom he was very close). We asked a Rav about using the name anyway. He told us to ask my mother if she would be upset if we used it. My mother, bless her, said that it wasn't an issue and that we should use it if we want. So, we were in the clear to use the name if we wanted to.
Well, it turned out it was a boy, so the whole matter was academic. When we finally did have a girl, we both decided that *we* weren't comfortable with it after all, and gave her a different name.
Likewise, my DH's grandfather (no longer living) has the same name as my father and his father. While we would like to name for DH's grandfather, we just know that we can't and that's that. We remember him fondly, but we just won't name after him. We'll have to leave that to cousins.
