If you are saying tehillim for someone specific do you just have them in mind? Say it out loud? Something else?

Before I start tehillim, I say the yehi ratzon with the specific person's full Hebrew name, and then commence.

I for one, before I start I say, my tehillim saying now should be a zchus for....and say name. For....and say cause cause-refua, shidduch, yeshua etc. During, I try to keep the person in mind. And if I lose my thoughts, I stop and get my thoughts together once again, and then resume.
