I'm probably going to be attacked by the "never formula" crowd, but at this point, if your baby weighs the same amount at birth , its time to give formula.
That's not to say that your milk isn't sufficient- I don't know and neither do you.
But your objective now is for your baby to gain weight. And when you look at the back of the formula can, it shows the precise amount of calories your baby will inject with each scoop.
After some time, if your baby still isn't gaining enough weight you are probably looking at a medical condition.
A family member of mine went through something similar. Her baby was pathetically tiny, and not gaining weight, so she started mixing her milk with formula, and still- nothing doing. Her Dr. was an idiot and was the wait and see kind . She took initiative, went to the emergency room because she knew she couldn't afford to wait for a specialist appointment.
Turns out- he had a medical condition that prevented him from gaining weight and he had surgery for a life threatening condition a few days later.
He's a newborn now and under the care of a GI. She adds rice cereal to every bottle and adds an extra scoop of formula to the bottle without the extra water.
I'm not here to scare you- in this story, it turns out it wasn't the mother's milk that was at fault. But- and here's the big but- The only way she knew it wasn't a calories thing was because she started giving formula.
And yes, she got herself a new pediatrician.
Start calling for a GI appointment now , or go to the emergency room, its faster.

This is what I do as well, but I use oatmeal instead of rice. I was fully bottle feeding before 6 months, I don’t remember exactly when though. I started off nursing, then was nursing and pumping and giving whatever I pumped. Then when that wasn’t enough I stopped pumping and started supplementing with formula, and then he just stopped nursing.
I wouldn’t recommend for OP to start playing around with what goes into a bottle on her own, that needs to be done under the guidance of a very good doctor. She said she has appointments tomorrow and Monday, so I’m hoping one of those is with a GI. Small Beans, if for any reason you need a GI recommendation, I will gladly give you our doctor’s info. He’s a little bit far from you (you’re in Lakewood, right?) but it might be well worth the drive.

I'm seeing Dr Albert for cardiologist and Dr Marshall for endocrinologist. Cardiologist had a cancelation and I already had appointment with endocrinologist for a different child, they let me add him

I know what gi, I would use but based on my baby I'm pretty confident it's not GI related. I will see what endocrinologist says on Monday, if he thinks I need a gi, I will schedule appointment then.

I truly hope you get to the bottom of this VERY quickly, and it turns out to be nothing major, something easily dealt with.

Sometimes the ml stay, but not always. There are some weeks when we see the same pre-feeding weight just about every day, maybe at the very end of the day it goes up, but otherwise it doesn't. We had one of those last week. This week, we had two days when we called baby "mister taryag" because he was 6.13 kilo before pretty much every feeding. Then last night it was 6.17, then 6.17 again this morning, then 6.19 this afternoon.

I can't make sense of when it does rise and when it doesn't but I've learned to concentrate on the amount baby takes in instead of on the pre-feeding weight. Though I will admit that when it stays the EXACT SAME number pre-feeding for a whole week straight, I do start to worry some. But we have been at this for a few months so I've learned that sometimes it rises and sometimes it just goes back to exactly where it was before the last feeding.

The only time I really start to stress is when the pre-feeding number drops more than 1-2 feedings in a row (ex. morning: 5.78, mid-morning: 5.78, lunchtime, 5.76, mid-afernoon - 5.75, evening 5.73 - then I get worried. But if it were exactly the same except that evening was 5.75/6 then I would not worry, I'd only worry if the next day followed a similar pattern).

Thank you. Yeah some days I have that where every feeding the pre feeding number is either the same or lower. Today he seems to be retaining some. I am keeping a diary, so hopefully I can find a pattern.

Just an update for those curious. The endocrinologist wants to corroborate with a GI and do testing together. He is making me an appointment at a GI for me, so I get one quickly.

I intended to update but life got busy.

I took him to an ENT and he said, no tounge tie and no other issues in his mouth.

I took him to a GI on Thursday, who said she believes it's allergies to my milk even though it is not coming up in the typical ways, just an absorption issue. She put him on elecare at 24 (normal is 20 calories) calories an ounce. She told me to go off, dairy and soy and pump for a week to wash my milk out and then come back. He's gained close to a pound in almost a week on elecare. Going back on Thursday, will hopefully update after.
