I want to take family leave on an intermittent basis. I went online to fill out a form and I’m so confused. Do I keep track of the days I take off an submit the whole form at the end? It says that venifits won’t be paid out after the date I sign

I did it intermittently. Each time I took a few days I’d send in a form and they loaded more money onto my card. You don’t have to wait till you finish all 56 days before filing for them.
I did it After my baby who is almost two, so not so long ago, and I haven’t heard that any rules changed
