I think the bigger concern is hitting someone upside their head (as the saying goes) with hard candies. A friend's shul only allows soft candies.
At my shul, some of the minyans do it, and some don't. At DS' bar mitzvah, we had boxes of lollipops to please give your whining child to keep him/her quiet while DS is leyning, but we didn't throw anything.

Same here. Unheard of not to throw for both bar mitzvas and shabbatot chatan.
If one of the parents and/or the couple is sefardi/edot hamizrach we will often also say a piyut.
