I want to buy my OB a present, although I am very bad at writing the inscription. Can anyone help with what to write inside a siddur?

That's so sweet. I don't remember feeling like giving my midwife a gift. Here are some ideas of what to write, Thank you so much for helping bringing (baby"s name) into this world. Your manner and behavior helped during this time.
