I go up half a size and have fun buying tons of Robert Zur and Sam Edelman flats in so many colors. Look for them on sale since they can be pricey .

sorry to bump this up from a while ago -
but what do people do if they have narrow feet towards the heel, with wider spread at the toes.
I can't wear wide shoes, but if I wear regular shoes, my toes are compressed!

That’s what I have. I find that sandals, sling backs, tie up and boots work best. Flats are difficult.

I find that flats (and pumps) when wide enough in front, fall off in the back. And I’ve tried so many. But if there’s a strap, you can adjust it and it holds onto your foot.
