Anyone have a template of what I need to write for vaccinations?

I wrote that it is my sincerely held religious belief that it is forbidden to take Covid Vax because the Torah says " you should guard your health exceedingly" and the vaccine is risky.

The Torah also says that if there is doubt

"Sit and don't do" and the covid vaccine is controversial.

I (first and last name), the parent of (child first and last name) request a religious exemption as I object to the vaccination of my child, as the proposed immunizations interfere with the free exercise of my child's religious rights.

The nj doh accepted this (my school actually sent it to me!)
